29 CFR 1926.602(c)(1)(viii)(B): Means were not provided whereby personnel on the platform could shut off power to the truck: a) On or about 03/08/2016 - 333 Haslam Street, Piedmont, AL, an employee was raised to the roofing surface by a Manitou powered industrial truck (PIT) and was observed getting out of the basket and getting back into the basket while the operator of the (PIT) dismounted the (PIT), left the (PIT) running, and didn't install any blocks behind the tires to leave the construction site to obtain water for the employees.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) On or about 03/08/2016 - 333 Haslam Street, Piedmont, AL, an employee working at 18 feet above the ground were unprotected from falls while gluing down fiber material. b) On or about 03/08/2016 - 333 Haslam Street, Piedmont, AL, employees working at 16 feet above the ground were unprotected from falls while rolling out insulation. c) On or about 03/08/2016 - 333 Haslam Street, Piedmont, AL, an employee working at 16 feet above the ground was unprotected from falls while routing electrical cords.
