29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. At the worksite where employees used tools and equipment with electrical cords and plugs, an inspection of the cords and plugs was not conducted before each day's use, thereby exposing the employees to an electrical contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. In the hallway of area A1 where employees were using a work light for illumination, the ground prong was missing from the plug, thereby exposing the employee to a 121 volt electrical contact hazard. b. In area A1 room 1109, a dehumidifier was plugged into an extension cord that was missing the ground prong, thereby exposing employees to a 121 volt electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the hallway of area A1 where employees were using a work light for illumination, the strain relief was pulled away from the outer jacket of the cord exposing the interior wires, thereby exposing the employee to a 121 volt electrical contact hazard.
