29 CFR 1910.213(g)(1):     Swing cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw:    On or about March 10, 2016 employees working at and around the chop saw station were exposed to amputation hazards during operation and the employer did not ensure that the saw blade was completely enclosed at the in-feed/out-feed openings and at the drive shaft cut-out.

29 CFR 1910.219(c)(2)(i):     All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    On or about March 10, 2016 employees operating and working around the chop saw station were exposed to becoming entangled in the saw blade's spinning drive shaft (arbor) that was not guarded along approximately 18 inches in front of the operators' control panel.

29 CFR 1910.219(c)(4)(i):   Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one-half the diameter of the shaft:  On or about March 10, 2016 employees operating and working around the chop saw station were exposed to becoming entangled in a rotating shaft approximately 3 inches long by 1 inch in diameter attached to the electric motor near the floor that once power the out-feed rollers/conveyor.

29 CFR 1910.219(f)(3):   Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about March 10, 2016 employees working at and around the chop saw station were exposed in-running nip points and to becoming entangled in a chain and sprockets:  a. A 3.5 inch sprocket and chain powering the in-feed rollers at the front of the chop saw table approximately 2 feet off the floor was not guarded.  b. A 2 inch sprocket was spinning and not guarded on the unused out-feed conveyor motor approximately 1 foot from the floor located at the end of the chop saw's table.
