29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:     (a)  J.A. Construction, at 1503 S. Galena Way, Denver, CO 8027: On and before March 11, 2016, the exposing employer did not ensure that employees working on a pump jack scaffold were no more than 14 inches from the face of the work, or that guard rail system or fall protective measures had been put into place. Condition exposed employees to fall hazards of approximately 18 feet while working on the scaffold.

29 CFR 1926.451(g)(4)(i): Guardrail systems where not installed along all open sides and ends of platforms on scaffolding:  (a)  J.A. Construction, at 1503 S. Galena Way, Denver, CO 80247: On an before March 11, 2016, the exposing employer did not ensure that guardrail systems where installed along all open sides of a pump jack scaffold system.  Specifically, two employees were observed working on a pump jack scaffold system with the center of the pump jack scaffold system missing approximately 4 feet of top and mid rails.  The scaffold was missing endrails.  Condition exposed employees working on the scaffold to fall hazards of approximately 18 feet.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)   J.A. Construction, at 1503 S. Galena Way, Denver, CO 8027: On and before March 11, 2016, the exposing employer did not ensure that employees were protected from injury, in that an employee walked near the rake edge of the roof, with a roof pitch of approximately 6-in-12, on a two-story wood-framed building without the use of a fall protection system. Condition exposed the employee to a fall of approximately 20 feet.
