Specifically, two employees were observed working on a pump jack scaffold system with the center of the pump jack scaffold system missing approximately 4 feet of top and mid rails.  The scaffold was missing endrails.  Condition exposed employees working on the scaffold to fall hazards of approximately 18 feet.
