29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       (a)  127 Park Place Drive, Sinking Springs, PA - Employees were observed using an aluminum extension ladder that did not extend at least 3 feet above the landing surface to the building, thereby exposing employees to a fall hazard of approximately 19 feet to the ground below, on or about March 11, 2016.      No abatement certification or documentation required.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 127 Park Place Drive, Sinking Springs, PA - Employee(s) were observed performing roofing work activity on a 7 in 12 pitch residential roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 19 feet to the ground below, on or about March 11, 2016.         No abatement certification or documentation required.    Fisher's Roofing & Siding Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 314235722, citation number 1, item number 1a and was affirmed as a final order on March 4, 2013, with respect to a workplace located at 1610 Wilmington Pike, Chadds Ford, PA 19317.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a)  127 Park Place Drive Sinking Springs, PA - The employee was performing roofing work activity on a residential roof at height of approximately 19 feet where the employees were not protected by a fall protection system, and the employer did not provide fall protection training to the employees to recognize a fall hazard, thereby exposing employees to serious injury, on or about March 11, 2016.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.    Fisher's Roofing & Siding Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.503(a)(1), which was contained in OSHA inspection number 314235722, citation number 1, item number 1b and was affirmed as a final order on March 4, 2013, with respect to a workplace located at 1610 Wilmington Pike, Chadds Ford, PA 19317.
