29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift:  On or about March 3, 2016, at the above addressed jobsite, an employee was exposed to a 29-foot fall hazard when he used an aerial lift to access a roof without means of fall protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (1)On or about March 3, 2016, at the above address jobsite, an employee was exposed to a 10-foot fall hazard when performing commercial roofing operations on a steep roof without means of fall protection.   (2) On or about March 3, 2016, at the above addressed jobsite, employees were exposed to a 7-foot fall hazard when performing commercial roofing operations on a steep roof without means of fall protection.
