29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On or about 3/22/2021, at 1618 N. Roosevelt Highway, Key West, Florida, employees were using a nail gun to attach the tiki leaves on the side of a tiki hut were exposed to a struck-by hazard  due to lack of eye protection.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   On or about 3/22/2021, at 1618 N. Roosevelt Highway, Key West, Florida, the employer did not conduct frequent and regular inspections of the job site to ensure employees wear eye protection while using  a nail gun to attach the tiki leaves on the side of a tiki hut.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about 3/22/2021, at 1618 N. Roosevelt Highway, Key West, Florida, employees were installing tiki leaves on the side of a tiki hut and were exposed to  a fall hazard of approximately 18ft due to lack of fall protection.
