29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) Warehouse, East Wall: On or about March 15, 2016 and at times prior, the employer exposed employees to struck-by and crushed-by hazards in that modifications or additions were performed without manufacturers prior written approval when cardboard was placed on top of the overhead guard.
