29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the items of this standard: a) Pleating Machine: On or about August 9, 2016 and at times prior thereto, the employer did not ensure the machine specific procedures identified the level of each hazardous energy source, the location of each energy isolation device, and the steps to complete the lockout tagout which exposed employees to amputations and other injuries associated with working on energized machinery.
