29 CFR 1910.269(p)(2)(i): Mobile equipment, when provided with outriggers, were not operated with the outriggers extended and firmly set, except as provided in paragraph (p)(2)(iii) of this section. a) Nelson Tree Service, Inc. on or about March 8, 2016, at the site located near the address of 1085 Gulf Rd. Elyria, OH: The employer failed to ensure the foundation/footing on which the Altec Truck Mounted Platform Model LR760-E70 outrigger was placed was capable of supporting the load. An outrigger pad positioned under the right outrigger shoe was bridged between both a concrete curb and adjacent uncompacted fill soil. While under load, the outrigger shoe and pad shifted from the rigid curb to the softer soil creating a condition that allowed the shoe to penetrate through the pad into the soil. The stability of the unit was compromised due to the condition, allowing the vehicle to overturn while an employee was performing line clearing activities from the extended platform. The exposed employee rode the platform approximately 30 ft. to the ground below.
