29 CFR 1917.45(e)(1): Exposed moving parts, such as gears, chains, and chain sprockets, that present a hazard to employees during crane and derrick operations, were not securely guarded: or in the alternative 29 CFR 1917.151(b)(8): Machines were not maintained in a safe working condition: (a) The Baldor drive motor coupling of the #4 Hoist was missing the guard, exposing employees to serious hazards of rotating parts.

29 CFR 1917.45(f)(1)(i): Crane or derrick operating controls were not clearly marked and no chart was posted at the operator's position indicating their function. (a) The control lever for the number 4 dock hoist was not marked to indicate the lever's operational functions. The Pacific Seafood was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1917.45(f)(1)(i), which was contained in OSHA inspection number 1118100, citation number 2, item number 5 and was affirmed as a final order on February 18, 2016, with respect to a workplace located at Charleston, OR 97420.
