29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 16, 2016, during the installation of wooden roof trusses for the construction of a three story apartment building, the employer did not ensure employees were protected from fall hazards of approximately 30 feet (framing top plate to the ground) and approximately 9 feet (framing top plate to the floor) by utilizing a guardrail system, safety new system, and/or other fall protection system or device.

29 CFR 1926.1424(a)(2)(ii): 29 CFR 1926.1424(a)(2)(ii): To prevent employees from entering hazard areas, the employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas and/or did not train each employee to understand what these markings signify. On or about March 16, 2016, while working with a mobile crane to install wooden roof trusses for the construction of an apartment building, the employer did not ensure employees were protected from struck by and pinch/crush hazards created by the swing radius of the mobile crane superstructure, by erecting and maintaining control lines, warning lines, or other similar barriers.
