The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: A. At the Core Blade department- the Respondent failed to ensure equipment was adjusted as required by equipment manufacturer for Wilton disc sander, Model # 4400A, serial # 06060148, exposing employees to injuries.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: On or about March 9, 2016, an employee was hospitalized from a work related injury and the company failed to report the accident to OSHA within 24 hours.
