29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about March 16, 2016, two employees used a JLG Sky Trak forklift truck as an elevated working platform. The employees were not trained to recognize, control or minimize hazards to conduct framing operations from this type of scaffold system, exposing the employees to a 15-foot fall hazard.
