29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a. On or about August 15, 2016, right side of home, the employee installing siding from a ladder jack scaffold system was not protected from falling 12 feet to the ground by the use of a personal fall arrest system.  b. On or about August 15, 2016, back of home, the employees installing siding from a ladder jack scaffold system were not protected from falling 14 feet to the ground by the use of a personal fall arrest system.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a. On or about August 15, 2016, the employee cutting siding and throwing it up to the employee on the scaffold system was not wearing a hard hat and was being exposed to a head injury.   Northwest Bay Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1132145, citation number 01, item number 001 and was affirmed as a final order on May 27, 2016, with respect to a workplace located at Lot 185, Cannonball Lane, Watersound, Florida 32413.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a. On or about August 15, 2016, the employee using a Makita saw to cut pieces of siding was not wearing safety glasses and being exposed to eye injuries from flying debris.  Northwest Bay Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1132145 , citation number 01, item number 002 and was affirmed as a final order on May 27, 2016, with respect to a workplace located at Lot 185, Cannonball Lane, Watersound, Florida 32413.
