Capacity, operation, and maintenance instruction plates, tags, or decals of the powered industrial truck were not changed accordingly: A.) New Castle Cold Storage, Inc. - On or about 3/16/16, and at times prior thereto, a man basket attachment was used on forklifts including, but not limited to, Yale forklift (serial # A883NO2618D) without the forklift manufacturer's written approval. Capacity plates and tags were not changed on the forklift to identify the attachment. The man basket attachment was used for activities that included, but were not limited to, changing light bulbs.

Forklifts included, but were not limited to, Yale forklift (Serial #A883N02618D) which had exposed copper on the charging cables. B.) New Castle Cold Storage, Inc. - On or about 3/16/16, and at times prior thereto, cables for the forklift charger were damaged. These chargers were located in areas that included, but were not limited to, dock areas.
