29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.  Capacity, operation, and maintenance instruction plates, tags, or decals of the powered industrial truck were not changed accordingly:      A.)  New Castle Cold Storage, Inc. - On or about 3/16/16, and at times prior thereto,  a man basket attachment was used on forklifts including, but not limited to, Yale forklift (serial # A883NO2618D) without the forklift manufacturer's written approval.   Capacity plates and tags were not changed on the forklift to identify the attachment.  The man basket attachment was used for activities that included, but were not limited to, changing light bulbs.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   A.)  New Castle Cold Storage, Inc. - On or about 3/16/16, and at times prior thereto,  the load capacity and data plates were not modified/changed on the Yale forklift (serial #A883NO2618D) to reflect the man basket attachment.  The man basket attachment was utilized on forklift including, but not limited to, the Yale forklift (serial #A883NO2618D).

29 CFR 1910.303(b)(7)(iv): Electric equipment shall be installed in a neat and workmanlike manner.  There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, or cut:          A.)  New Castle Cold Storage, Inc. - On or about 3/16/16, and at times prior thereto, forklifts had damage to the battery charging cables.  Forklifts included, but were not limited to, Yale forklift (Serial #A883N02618D) which had exposed copper on the charging cables.    B.)  New Castle Cold Storage, Inc.  - On or about 3/16/16, and at times prior thereto, cables for the forklift charger were damaged.  These chargers were located in areas that included, but were not limited to, dock areas.

The items blocking access to the panel boxes included, but were not limited to, pallets, boxes and tarps.
