29 CFR  1910.37(a)(4):Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:   A.) New Castle Cold Storage, Inc., New Building, on or about 4/15/25 and times prior thereto: the Exit sign to the door in the freezer had been damaged and no longer clearly indicated the presence of an exit.      B.) New Castle Cold Storage, Inc., New Building, on or about 4/15/25 and times prior thereto: Ice was built up on the panic bar of the exit door.  The panic bar was not able to be opened without excessive force.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  A.) New Castle Cold Storage, Inc., Back dock, on or about 4/15/25 and times prior thereto:  A compressed gas cylinder of nitrogen  on the scissor lift work platform  was not in a secure upright position, was laying on its side, and not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). New Castle Cold Storage, Inc., New Building, on or about 4/15/25 and times prior thereto: Portable multi-purpose dry chemical fire extinguishers were distributed throughout the facility for employees to use in the event of an incipient stage fire. New Castle Cold Storage, Inc., Back Dock, on or about 4/15/25 and times prior thereto: two knockout box electrical receptacles were stacked on top of each other at dock 8.  There was space between the boxes that was open exposing employees to unguarded live parts. The forklift was not tagged or posted with signs to alert employees that there was a problem with the forklift and that it must not be used.
