29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 16, 2016, employees were exposed to the rotating parts on autoclave #11 when the employer did not ensure employees performed the procedures for the application of energy control by; preparing the machine for shutdown [per the 1910.147[(d)(2)], isolating the machine of all energy sources [d)(3)], affixing lockout devices to hold the machine in the off position [(d)(4)(i)], and verifying that isolation and de-energization of the machine had been accomplished [(d)(6)], during the cleaning of the interior wheel. Energy sources include, but not limited to, electrical and pneumatic power.
