U.S. Pipe Fabrication, LLC, Gainesville, GA - On 2/16/16, an employee experienced a broken and a cut finger that required medical treatment and restricted activities. The injury was not recorded in the OSHA 300 log within seven days of the injury on or about 3/15/16.

The written respiratory was not tailored to the respirators used at the site, on or about 4/29/15.
