29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment.  On 3/7/16, at the Gainesville, Georgia facility, the employer did not have each authorized employee performing service and maintenance to equipment affix their own lockout lock to the equipment on which work was being performed. i.e. Authorized employees with locks were locking out for other employees who were not provided locks by the employer (and had not been trained as designated Authorized employees).

Custom Fab, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii)(b), which was contained in OSHA inspection number 1064509, citation number 2, item number 1 and was affirmed as a final order on 2/18/2016, with respect to a workplace located at 1101 Industrial Blvd., Gainesville, Georgia.
