OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, including burns and lung damage, in that employees were exposed to fire and pulmonary system health hazards from ozone generated and used at the facility.      a) Evaporator Room, employees operating and maintaining an ozone generation system were exposed to fire and health hazards of ozone in work areas from ozone piping systems without secondary containment, from the lack of ventilation in the ozone generator cabinet, and from the use of ozone in a gas fired waste sludge evaporator, on or about, March 16, 2016.      "ABATEMENT CERTIFICATION REQUIRED"

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and (h)(2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced.      a) Evaporator Room, employees were exposed to ozone from an ozone generation system and were not provided effective information and training at the time of the system installation, on or about, March 16, 2016.    "ABATEMENT OBSERVED DURING INSPECTION - NO CERTIFICATION REQUIRED"
