29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    On or about March 16, 2016, the employer failed to ensure that the open sided platform located in building 11 of the facility was guarded with standard railings. The platform divides the facility into two sections and is identifiable as a former loading dock. Half of the platform length contained no guardrails.

29 CFR 1910.23(e)(1): A standard railing did not consist of a top rail, intermediate rail, and post, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor.  The intermediate rail was not half way between the top rail and the floor, platform, runway, or ramp.  On or about March 16, 2016, the employer failed to ensure that the open sided platform located in building 11 of the facility contained an intermediate rail. The platform divides the facility and is identified as a former loading dock.

A flammable propane tank and plastic gasoline tank were stored next to the main exit door of building 11 at the facility.    b) On or about March 16, 2016 the employer failed to ensure that flammables were not stored near the exit route/ door. Diesel tanks were stored directly outside the north emergency exit door.
