29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about January 10, 2017, the employer failed to provide a certification record for the monthly inspection of hoist chains used in the facility. The employer had performed the inspection but did not maintain record of certification.
