29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Facility wide, B. & R. Machine, Incorporated, 305a Moody Street, Ludlow, Massachusetts; Electrical equipment was used facility wide outside of its listing and labeling to provide power to industrial and personal equipment which exposed employees to electric shock and fire hazards. a. Relocatable power taps, aka power strips, were used in the facility for industrial and personal equipment. There were multiple instances where they were plugged in series with extension cords and multiple outlet adapters. The UL listing and labeling states that these items are for low amperage equipment only such as computers, printers, monitors, and other electronic office equipment. The listing also requires that the relocatable power taps be plugged directly into a wall outlet. b. Multiple outlet adapters were used to power equipment, extension cords, and relocatable power taps throughout the production area. The UL listing and labeling for these items is for residential use only.
