29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 290 Moody St, Ludlow, MA On December 18, 2017 and at times prior, the employer had not documented a procedure and employees did not use an energy control procedure when they set up and maintained a Kitamura HX250G Horizontal Machining Center. The newly installed machine had been in use for a few months and the employer had not documented a machine specific energy control procedure. On December 18, 2017, an employee who was setting up the energized machine received hand injuries in the ram section of the machine where a guard was not in place.

The procedures did not include the pneumatic energy source.
