29 CFR 1910.268(b)(7): The employer did not ensure that no employee approaches or takes any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2, or in paragraphs (b)(7)(i) through (b)(7)(iii) of this part: a) worksite near 116 East Gilmore Road in Grove City, PA. On or about March 15, 2016, employees were exposed to electrical shock hazards in that, the aerial lift encroached on the overhead electrical lines within the 24-inch approach distance of the 7.2 kV. Employees were operating a bucket truck to install fiber-optic cable along the utility poles. An employee was shocked when he contacted the electrical line while in the bucket truck.

29 CFR 1910.268(c)(3): Employers did not provide first aid training, including instruction in artificial respiration, required by this section: a) worksite near 116 East Gilmore Road in Grove City, PA. On or about March 15, 2016, employees installing fiber-optic cable were not given training on First Aid/ CPR. The employees were working from bucket trucks, around electrical lines, along roadways. Employees were exposed to fall hazards, electrical hazards and struck-by hazards.
