In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  a) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the nominal 208 V dead front panel mounted on a structural steel column (in front of Oven Press #7) was crushed by a forklift and was left energized in service without repair.   b) Plant, Airlay Building - On or about 03/08/16 and at times prior thereto the conduit protecting the power cable in the Laroche machine  was broken and separated in proximity of the strain relief device (480 V motor). Employees were exposed to potential loss of insulation and contact with 480 live parts. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Plant,  Airlay - On or about 03/08/16 and at times prior thereto the flexible cable carrying power from the regrinder  switch box  to the air blower switch box  was left unprotected and was not mounted inside  a conduit.  Employees were exposed to potential loss of insulation and contact with live parts.
