29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a. Located in Substation 1 at 400 Case Avenue in Newark, Ohio: On or about March 24, 2016 and at times prior, the M1F3 overcurrent relay for breaker T12 and T13, the 51N2 neutral relay for Bus 1, and the 53A neutral relay for Bus 3 and Bus 4 were not operating as intended by the manufacturer. The condition exposed employees to arc flash and blast hazards while performing work in the substation. b. Located in Substation 11 at 400 Case Avenue in Newark, Ohio: On or about March 24, 2016 and at times prior, the 480 volt Washwater Tie Breaker 11F1 was not operating as intended by the manufacturer due to a defective trip mechanism. The condition exposed employees to arc flash and blast hazards while performing work in the substation. c. Located in Substation 27 at 400 Case Avenue in Newark, Ohio: On or about March 24, 2016 and at times prior, three 480 volt breakers, 27F1, 27F2, and 27F4 were not operating as intended by the manufacturer due to defective tripping mechanisms. The condition exposed employees to arc flash and blast hazards while performing work in the substation.

Located in the Wool Plant at 400 Case Avenue in Newark, Ohio: On or about March 17, 2016 and at times prior, the employer did not ensure that the Bagger F6 Motor Control Center (MCC) had arc flash warning labels to warn employees of the potential arch flash and blast hazards.
