29 CFR 1910.1030(g)(2)(vii)(N): The bloodborne pathogens training program did not contain an opportunity for interactive questions or answers with the person conducting the training session.  On or about 03/16/2016, employees working various tours at the Department of Army USA MEDDAC Blanchfield Army Community Hospital located at 650 Joel Drive on Fort Campbell, KY, did not have immediate access to someone to answer their bloodborne pathogen questions during training.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical.   a) On or about 03/16/2016, employees working in the Army USA MEDDAC Blanchfield Army Community hospital located at 650 Joel Drive on Fort Campbell, KY, were exposed to hazardous materials (eye, skin, and/or respiratory irritants) for which the employer did not maintain safety data sheets for each hazardous material used as follows:   1 The Dental Laboratory employees were using Vinyl All Purpose Conditioner (flammable); Burning Alcohol (Denatured Alcohol) (Poison, flammable liquid); Hager EconoGas (Extreme Flammable, eye, skin, respiratory irritant); Hygenic Green Endo ICE Refrigerant Spray (can cause heart irregularities).    2. The Canteen employees used Bio-Matic Waste Liquefier Odor Eliminator (dermatitis and skin sensitizer and may cause eye irritation; and 3M Stainless Steel Cleaner & Polisher (eye, skin, respiratory tract irritant)  3. The Brace Shop employees use acetone (eye, skin, respiratory irritant), propane gas (flammable, can cause burns), and steris coverage spray (eye, skin irritant)  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On or about 03/16/2016, the Department of Army USA MEDDAC Blanchfield Army Community hospital located at 650 Joel Drive on Fort Campbell, KY, did not post, from February 1, a OSHA 300A Form (annual summary) for calendar year 2015.

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  a) On or about 03/16/2016, employees working in the Dental Lab at the Department of Army USA MEDDAC Blanchfield Army Community hospital located at 650 Joel Drive on Fort Campbell, KY, were exposed to hazardous materials (eye, skin, and/or respiratory irritants) for which the employer did not compile a complete chemical list to include the following hazardous materials used:  1 The Dental Laboratory employees were using Vinyl All Purpose Conditioner (flammable); Burning Alcohol (Denatured Alcohol) (Poison, flammable liquid); Hager EconoGas (Extreme Flammable, eye, skin, respiratory irritant); Hygenic Green Endo ICE Refrigerant Spray (can cause heart irregularities).    2.
