29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) On or about March 14, 2016 One employee was using pneumatic nail guns to secure roof trusses to the top plates of the structure and was not protected by eye injury from the sudden impact of nails and wood chips because he was not wearing proper safety glasses.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a)  On or about March 14, 2016 employees using circular hand held saws were exposed to electrical shock hazards because the flexible extensions cords they were using did not have a continuous path to ground because the ground pins on the cords had been broken off or removed.

29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a. The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on March 14, 2016, when employees are working on a residential 6:12 pitch roof truss slope, at 103 Maiden Lane, Ponte Vedra Beach, FL without fall protection and were exposed to falls of approximately 9 feet.
