OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to dust explosion, deflagration, or other fire hazards as the result of working at or near dust collection systems handling combustible toner compound/additive dust which were not designed/installed/located to prevent or minimize exposure in the event of a deflagration, explosion or other uncontrolled fire event: (a) Facility, Production Area, Latrobe, PA: On or about 03/24/16 PVC piping was used as ductwork to collect and move combustible dust fines from the blender to the shaker during the toner making process. (b) Facility, Production Area, Latrobe, PA: On or 03/17/16 the Donaldson Torit collection systems installed and used on mill 1, mill 2 and classifiers 4 and 5 were located inside the building.
