29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:    (a)Raven Industries, Inc. at 811 West Algonquin Street, Sioux Falls, SD 57104:  On or about and at times prior to November 30, 2018, the host employer did not ensure a temporary employee attempting to perform a cutover cycle on the Battenfeld-Gloucester Winder Model # 144WNR, Serial # 39095-01 located on Line #3 in Extrusion, was protected from the point of operation. An employee was caught-by and wrapped up in a rotating spindle while attempting to transfer industrial plastic film sheeting by stapling and cutting the film from a full roll to an empty core roll. The employee broke the horizontal plane of the light curtain attempting to make the transfer and entered the point of operation while the spindle continued to rotate or spin down, decelerating and eventually coming to a stop. As the spindle was decelerating, the employee attempting to make a cut with a utility knife to proceed with the transfer to the new core and was caught-by the film and pulled into the new roll. This hazard exposed the employees to caught-by and caught-in-between hazards.      Abatement Note: The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.
