29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Strip Cutting Area in the northwest corner of the UNICOR building: Two (2) TRC 1000 AA cold strip cutter machines did not have guarding to prevent the operators from making contact with the shear type blade while automatically cutting strips of materials for belt loops.   b) Strip Cutting Area in the northwest corner of the UNICOR building: Two (2) TRC 1000 AA hot strip cutter machines did not have guarding to prevent the operators from making contact with the heated cutter while automatically cutting strips of materials for belt loops.   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) North wall of the Welding Shop Supervisors Office: A Belkin relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, TVs, stereos, printers, etc. was used to proved power for high powered loads such as a commercial grade refrigerator and a Sharp microwave oven.

29 CFR 1910.304(b)(2)(ii): Receptacles having grounding contacts did not have contacts that were effectively grounded.  a) Southeast end of the work floor of the UNICOR building: A receptacle outlet located near the ceiling above a QV Inspection Station supplied power to the station for lighting and equipment was found to have a false ground.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) South end of the Welding Shop: The 120 volt electrical cord on a Dayton pedestal fan was missing the ground pin on the end of its' plug-end.    b) South wall of the UNICOR Warehouse: The 120 volt electrical cord on a  Nova pedestal fan was missing the ground pin on the end of the plug-end.   c) Center of the Front Lobby: The 120 volt electrical cord on a  Oreck vacuum cleaner used to clean the carpet was found to have a missing ground pin on the plug-end.   d) East end of the Laundry Room: The 120 volt electrical cord on a  Sanitaire Air Mover was found to have a missing ground pin on the plug-end.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) QA Station northwest corner of UNICOR work floor: Due to lack of effective visual inspections a 120 volt flexible cord providing power to a fluorescent light was found to be tied into a knot, and was also showing visible damage to the outer insulation showing the insulated wired beneath.

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:  a) South wall of the Camp Recreation Room: Due to the cover over the mechanical/electrical parts of a water cooler having been removed it exposed employees to the unguarded fan blade located inside.
