29 CFR 1910.242(a): The employer did not ensure that employees were furnished with tools and equipment that were in a safe condition:  (a) Quality Associates Inc. - The employer provided its own manual (human-propelled) utility carts for transporting the glue pot to set up production lines.  The utility carts had no handles or other safe means for employees to use to grasp the carts, employees grasped the side of the glue pot and the extended metal support frame that extended 9 feet 7 inches high from the floor of the carts when moving the carts, exposing employees to the hazards of the employee pushing the utility cart over, thereby exposing employees to injuries, on or about March 18, 2016.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
