29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: (a) Front side, new residential construction located at 11045 Callaway Drive, Johns Creek, GA 30097: Employee was exposed to severe laceration and amputation hazards while cutting bricks with a DeWalt (Model - D28715) Chop Saw with the upper guard in the fixed open position. The saw was modified with a wire tied to the handle which prevented the guard from retracting automatically when a cut was made.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: (a) Front porch roof, new residential construction located at 11045 Callaway Drive, Johns Creek, GA 30097: Employee was exposed to falls of approximately 19-feet, 7-inches to the lower level while engaged in masonry activities on a tubular welded scaffold platform with no fall protection system in place. The employer failed to erect guardrails along all open sides of the working platform. (b) Front side, new residential construction located at 11045 Callaway Drive, Johns Creek, GA 30097: Employee was exposed to falls of approximately 14-feet, 5-inches while loading bricks onto the scaffold platform with no fall protection system in place. The employer failed to erect guardrails along all open sides of the work platform.
