29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about March 21, 2016 at 309 Green Island Road, Bonaire, Georgia, an employee was exposed to a fall hazard of approximately eleven (11) feet to the lower level when working from a 12/6 pitched roof installing vinyl siding flashing without any means of preventing falling to the lower level.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   On or about March 21, 2016 at 309 Green Island Road, Bonaire, Georgia, an employee working on a 12/6 pitch roof was exposed to a fall hazard of eleven (11) to the lower level when using a section of an aluminum extension ladder to prevent slippage while installing vinyl flashing.
