29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 11, 2021, at 438 Patina Drive in Montgomery, TX, two employees were exposed to an injury while operating nail guns without wearing eye protection.

29 CFR  1926.451(b)(3):Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  On or about June 11, 2021, at 438 Patina Drive in Montgomery, TX, an employee was exposed to a fall hazard while working on a pump jack scaffold whose front edge was more than 14 inches (36 cm) from the face of the work.

29 CFR  1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  On or about June 11, 2021, at 438 Patina Drive in Montgomery, TX, an employee was exposed to a fall hazard while working from a pump jack scaffold that did not have an adequate foundation on base plates and mud sills that would prevent the scaffold from settling into the ground.

An employee was working from a pump jack scaffold at about 13 ft. high. b.	An employee was working from a job-made wooden scaffold at about 20 ft. high.

29 CFR  1926.452(j)(2):Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.  On or about June 11, 2021, at 438 Patina Drive in Montgomery, TX, an employee was exposed to  fall hazard while working from a pump jack scaffold that was not secured by rigid bracing.
