29 CFR 1926.652(d)(1): Materials and equipment used for protective systems were not free from damage or defects that might impair their proper function: (a) Trench Box 83: On the morning of March 22nd, 2016 a competent person did not ensure that manufactured spreader bar pins were in place in spreader bars of the trench box that was in use. A spreader bar pin was not inserted fully through the openings of a lower spreader bar connection point and secured with a cotter pin. The spreader bar pin was to short and held in place with tape. (b) Trench Box 91: The trench box vertical channel welded to the inside walls of the trench box, and allows adjustable spreader bars to be adjusted up and down, had broken welds. Additionally, the trench box inside and outside walls had several large puncture holes that exposed the interior structure of the box.
