29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).   (a) On or about December 12, 2020, at the construction site located near 43'01'23.2"N 88'17'08.0"W, Meadowbrook Road, Waukesha WI , employees were attempting to remove hosing from between the trench box and the trench wall that was approximately 7 feet deep without adequate cave-in protection, thereby exposing the employee to a struck-by /crushing hazard.  Advance Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1482921, citation number 1, item number 1 and was affirmed as a final order on October 22, 2020 with respect to a workplace located at Bellvue Ave At Amy St., Green Bay, WI 54302.
