29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:  a) On or about March 22, 2016, and at times prior, employees in the shop were exposed to electrical hazards in the following instances:  1. The power cord for the Miller Millermatic 252 MIG welder was damaged, exposing the internal conductors; 2. The power cord for the Ryobi pedestal grinder had been repaired using only twist connectors, exposing the internal conductors; 3. Flexible extension cords throughout the shop used to supply power to hand grinders were damaged, exposing the internal conductors; 4. The outlet on the west wall of the shop used for the Black and Decker grinder was damaged, exposing live electrical parts.
