On or about March 6, 2018 and at times prior, employees of West Side Fabrication, LLC were exposed to this hazard while operating the Ryobi bench grinder which was missing a tongue guard.  West Side Fabrication, LLC was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number 1261379, Citation Number 3, Item Number 1b, and was affirmed as a final order on February 7, 2018, with respect to the same bench grinder at the same workplace located at 2120 Naegele Rd., Colorado Springs, CO.    West Side Fabrication LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1134089, citation number 2, item number 1b, and was affirmed as a final order on May 16, 2016, with respect to the same bench grinder at the same workplace located at 2120 Naegele Rd., Colorado Springs, CO.

On or about March 6, 2018 and at times prior, employees of West Side Fabrication, LLC were exposed to electrical hazards in that two circuit breaker panels in the main shop did not have all of the circuits marked at the overcurrent devices to indicate their purpose.    West Side Fabrication, LLC was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number 1261379, Citation Number 1, Item Number 4b, and was affirmed as a final order on February 7, 2018, with respect to the same breaker panels and the same workplace located at 2120 Naegele Road, Colorado Springs, CO.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables are not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  a) The employer, West Side Fabrication, LLC, does not ensure that flexible cords and cables are protected from damage by sharp edges of a metal wall hole.  On or about March 6, 2018 and at times prior, employees were exposed to electrical hazards while working inside the metal building where flexible cords passed through holes in metal walls between the interior and exterior of the building to provide power to evaporative coolers on the east side of the building.  The sharp edges of the metal wall could cause damage to the cords. On or about March 6, 2018 and at times prior, employees were exposed to this hazard while working in the vicinity of the Piranha ironworker multi-conductor cord that enters the panelboard with no device such as a bushing to prevent direct pull at the panel terminations.  b) The employer, West Side Fabrication, LLC, does not ensure that flexible cords are connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  On or about March 6, 2018 and at times prior, employees were exposed to this hazard while working in the vicinity of and operating the powered chain hoist without a strain relief device for the pendant. Toluene is a flammable and acutely toxic hazardous chemical that may cause moderate to severe irritation to the skin and eyes respectively, and may cause harmful central nervous system effects. Methyl Ethyl Ketone is a flammable chemical that may cause irritation to the eyes, skin, and respiratory system. On or about March 6, 2018 and at times prior, employees were exposed to hazardous chemicals without the benefit of a hazard communication program. On or about March 6, 2018 and at times prior, employees of West Side Fabrication, LLC were exposed to hazardous chemicals in the workplace without the benefit of the safety data sheets for those chemicals.  Employees handle chemicals, including but not limited to, Startex Toluene solvent and Startex Methyl Ethyl Ketone for which safety data sheets (SDS) are not readily available.  Toluene is a flammable and acutely toxic hazardous chemical that may cause moderate to severe irritation to the skin and eyes respectively, and may cause harmful central nervous system effects.  Methyl Ethyl Ketone is a flammable chemical that may cause irritation to the eyes, skin and respiratory system.
