29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 22, 2016, the employer did not ensure employees were protected while entering and exiting an aerial lift to access the roof of a commercial building. This condition exposed employees to a fall hazard above 6 feet.

29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set. On or about March 22, 2016, the employer did not ensure an aerial lift was fully lowered before the operator left the area. This condition exposed employees to struck-by hazards.
