29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about March 10, 2016:  a) Employees were exposed to fall hazards of approximately 25 feet while preforming blasting and painting operations on a suspended bridge without the use of fall protection means.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 10, 2016:  a) Employees were exposed to fall hazards while preforming bridge blasting and painting operations when the employer failed to ensure that each employee was properly trained in the ability to reduce or eliminate the fall hazard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On or about March 10, 2016:  a) The employer failed to ensure that OSHA was contacted and made aware of an employee being hospitalized due to injuries sustained in a fall from height. OSHA was made aware of the accident and hospitalization by a media referral on or about March 14, 2016.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
