29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a)SR 193 Northbound Bridge (CSX Railroad<Substructure<Rutgers Rd): On or about January 19, 2020, the employer required his employee(s) to perform highway bridge painting activities on a ladder whose side rails did not extend at least 3 feet (0.9 m) above the pier cap's landing surface and was not secured to a rigid support on the landing surface; exposing employee(s) to fall hazards of approximately 30 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) SR (State Route) 193 Northbound Bridge Mobile, AL (CSX Railroad<Substructure<Rutgers Rd): On or about January 19, 2020, the employer required his employee(s) to perform highway bridge painting containment activities without fall protection on the pier cap's landing surface; exposing employee(s) to fall hazards of approximately 30 feet to the ground below. The International Rigging Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1134325, citation number 1, item number 1 and was affirmed as a final order on August 29, 2016, with respect to a workplace located at SR53 Gainesville, GA 30507.
