29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. Guarding devices were not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a) Gate department- hydraulic roller bender (SN 0212027) - employees use the roller to bend metal and the metal and plastic guard that was on the machine was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle, on or about 3/23/16.  b) Gate department- manual roller bender (SN 2030356) - employees use the roller to bend metal and the machine did not have a guard to prevent the operator from having any part of his body in the danger zone during the operating cycle, on or about 3/18/16.   The Barrette Outdoor Living, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.212(a)(1)), which was contained in OSHA inspection number 928843, citation number 1, item number 5 and was affirmed as a final order on (1/10/14), with respect to a workplace located at 75 John B Brooks Road, Pendergrass, GA 30567.
