29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: The employer is failing to protect employees from the physical hazard of being struck-by a dock plate leveler. The specific energy control procedure for the dock plate leveler did not address the potential energy source of gravity. An employee was hit in the head with a dock plate that was blocked in place with a pallet which broke under the weight of the dock leveler, instead of identifying and using the proper equipment to hold the weight of the dock leveler. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed.
