Impact Employment Solutions, Inc. located in McComb, Ohio: On or about March 17, 2016, the employer did not ensure employees who were assigned by the host employer to clean conveyors were adequately trained to understand the difference between affected and authorized employees under the host employer's lockout program, exposing them too caught-in hazards.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a. Impact employment solutions, Inc. located in McComb, Ohio: On or about March 17, 2016, the employer did not ensure authorized employees applied lockout locks on the electrical disconnect for the oven 9 exit conveyor while cleaning the equipment. An employee who had only received training on the company's lockout program at the affected level was locking out the equipment.
