29 CFR 1926.501(b)(3):     Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:    a)  205 Route 94, Lafayette, NJ.    Employee was exposed to 24 foot falls to the ground below as the worker hoisting the bucket at the roof's edge was not using a personal fall arrest or guardrail system.                                          Violation observed on or about 03/23/2016.

29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a)  205 Route 94, Lafayette, NJ.    Employees working on the commercial roof were exposed to 24 foot falls to the inside floor below as workers were not protected from falling through skylights.                                            Violation observed on or about 03/23/2016.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a)  205 Route 94, Lafayette, NJ.    Employees were exposed to 24 foot falls to the ground below as they were not using an appropriate method of fall protection while working on the low sloped commercial building's roof.                                              Violation observed on or about 03/23/2016.      Stanley & Orke, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 900416, citation number 2, item number 1 and was affirmed as a final order on 06/07/2013, with respect to a workplace located at 200 East Main Street, Rockaway, NJ.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a)  205 Route 94, Lafayette, NJ.    Employees were exposed to approximately 12 foot falls to the roof below as the portable ladder's side rails did not extend at least 3 feet above the upper roof being accessed.                                            Violation observed on or about 03/23/2016.      Stanley & Orke, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 900416, citation number 1, item number 3 and was affirmed as a final order on 06/07/2013, with respect to a workplace located at 200 East Main Street, Rockaway, NJ.
