29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Jobsite-On or about May 3, 2016, employees were allowed to work performing roofing and framing activity without adequate fall protection, in that the employees were not connected in a way that would prevent impacting the lower surface, exposing employees to fall hazards up to 25 feet.      1) Employee installing wooden sheath was exposed to 12-foot fall hazard      2) Employee stabilizing 2 X 4 roof truss was exposed to 25-foot fall hazard

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Jobsite-On or about May 3, 2016 and at times prior to, the employer did not provide adequate fall protection training for employees who were working on roofing and framing activity wearing personal fall protection equipment deployed in a way that would still allow the employees to impact the lower surface in the event of a fall, exposing employees to fall hazards up to 12 feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) Jobsite-On or about May 3, 2016, employees used the top rungs of step ladders to gain access to the roof area to conduct roof framing activity and to install roof sheathing, exposing employees to a 12 foot fall hazard.
