29 CFR 1910.213(h)(3): Radial saw(s) were not provided with an adjustable stop to prevent the forward travel of the blade beyond the position necessary to complete the cut in repetitive operations: Employees using the 14 inch Delta radial arm saw were exposed to amputation hazards and the employer did not ensure that the saw's stop was adjusted to prevent the blade from traveling beyond the cutting table's edge.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Employees were exposed to approximately 208 volt electrical hazards: a. The electric panel mounted to a support beam in the shop area had an unused opening that did not prevent access to live conductors. b. The electric panel located at the southwest wall had two unused openings; one on the left bottom side, and one on the left underside. c. The electric panel mounted on the north wall near the three-head Baker band saw had an unused opening on the underside on the left.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about March 1, 2016 two employees were replacing the blade on the single head Baker band saw and only one lock had been used to isolate the machine's electrical energy when two employees replaced the saw's blade.
